Family Membership for 2

This membership is for two family members who wish to enjoy MDHCA membership together, such as spouses, partners, children and grandchildren.

• Both members receive the benefits of membership and may participate in Association activities.
• You will be asked to provide the name and contact information for the second member.
• This membership covers two individuals.
• This is an annual membership.
• You will receive renewal notices prior to the end of your membership term.
